Treating Perimenopause and Hypothyroid Fatigue in Salisbury, MD

A busy mom and wife in her early 50s came to Remedy Longevity & Cellular Medicine in Salisbury, MD with pronounced fatigue, no motivation, and an overall sluggish feeling. Labs revealed a hypofunctioning thyroid and the hormonal flux of the perimenopause-to-menopause transition. With thyroid replacement, an anti-inflammatory diet, ketone and peptide supports, and bioidentical hormone therapy (estradiol, progesterone, and testosterone), she now feels energized, mentally clear, and present for her family — and continues to work with her mental health provider as part of a coordinated approach.

The Full Story, in Our Clinician's Words

I had a female patient who was in her early 50s who came to me complaining of very pronounced fatigue, and she was having no motivation, no energy to get through her daily activities. She's a busy mom, she's a wife, and was just overall feeling sluggish. So of course, we ran labs on her. We found that her thyroid was acting very sluggish — just a very hypofunctioning thyroid, as evidenced by the labs that we saw. So we did put her on thyroid replacement. And we have put her on a very anti-inflammatory diet and another regimen including ketones and some peptide supports. And now the patient is feeling much more energized. She has the motivation and the wherewithal to go about her day and be present — the mom that she wants to be for her kids, the wife that she wants to be for her husband.

This patient also was unfortunately dealing with a lot of mental health issues at the time. She was feeling as though she was close to sort of a mental health breakdown. What we also saw in her lab work was that she was very much in this transition period between perimenopause and menopause, where she was still cycling, but her estrogen levels were very much given to flux from month to month, and this was contributing to significant symptoms for her. And so we did place her on bioidentical hormone therapy with some estradiol support, some progesterone, as well as testosterone. And now the patient is feeling as though her mental acuity and her cognitive wellness is in a much better place than it was when she first came in through our door. She's working with another mental health provider for counseling and therapy, and just feels as though she's in a much better spot today than she was initially.

Questions About This Case Study

Can thyroid problems and perimenopause happen at the same time?

Yes, and they often overlap in the 40s and 50s — both can cause fatigue, low motivation, and brain fog, which is why comprehensive lab work matters. This patient's labs showed both a sluggish thyroid and the estrogen fluctuations of the perimenopause-to-menopause transition, and each was addressed.

Can hormone changes affect mood and mental health?

Fluctuating estrogen during the menopause transition can contribute to significant mood and cognitive symptoms. In this case, bioidentical hormone therapy helped her mental acuity and cognitive wellness — while she continued counseling with a mental health provider. Hormone care complements, and never replaces, mental health treatment.

What does treatment for this kind of fatigue involve?

It starts with labs to find the actual drivers. For this patient that meant thyroid replacement, an anti-inflammatory diet, ketone and peptide supports, and bioidentical hormone therapy with estradiol, progesterone, and testosterone — a plan tailored to what her lab work showed.
